Payment Policies
Important Payment Information
- ✓ Copayments are due at the moment of service
- ✓ Balances due are expected to be paid prior to the next appointment
- ✓ Missed appointments without a 24-hour notice will be charged a $50 administrative fee
- ✓ 24-hour cancellation notice is required
- ✓ Two missed appointments in a row will result in discharge
- ✓ A fee will be charged for letters and medical records copies or transfers
Cancellation Policy
📅 24-Hour Notice Required
We require at least 24 hours' notice for appointment cancellations or rescheduling.
💰 Missed Appointment Fee
Missed appointments without proper notice will be charged a $50 administrative fee.
⚠️ Discharge Policy
Two consecutive missed appointments will result in discharge from the practice.
